Form and Email Legitimacy

Florida State University uses a web-based tool called Maxient. Maxient forms and messages from notifications@maxient.com are legitimate.

 

Forms for Students and Instructors

  • First Offense Form (PDF) and Instructions (PDF) - This form is used to document first offense resolutions between students and instructors. It should not be used when students have a prior record of academic misconduct or when the alleged violation is egregious (as defined in the Academic Honor Policy).
  • Second Offense or Egregious Offense Form - This form is used by instructors to refer cases to Academic Honor Policy hearings or Administrative Case Resolution meetings. It should be used when the student has a prior record of academic misconduct or when a first offense is egregious (as defined in the Academic Honor Policy).
  • Early Expungement Request Form - This form is used by students who have a single violation on their record and are within one year from graduating (determined and verified by office program-mapping documentation) to request a review to determine if their Academic Honor Policy record may be removed early from the confidential database.
  • Role of Advisor Request Form - This form is used by students to request to have an advisor or support person present during an Academic Honor Policy hearing or Administrative Case Resolution meeting.
  • Witness Request Form - This form is used by instructors and students to request witnesses attend an Academic Honor Policy hearing to provide a witness statement relevant to the allegation being considered.
  • Witness Statement Form - This form is used by witnesses to provide a written witness statement to be included in an Academic Honor Policy hearing.
  • Administrative Case Resolution Form - This form is used by students to waive their right to an Academic Honor Policy hearing and choose to resolve their case by Administrative Case Resolution meeting. This option requires permission from the instructor of record and the Office of Faculty Development and Advancement.
  • Appeal Request Form - This form is used by students to appeal the decision from an Academic Honor Policy hearing or Administrative Case Resolution meeting to the Academic Honor Policy Faculty Appellate Officer.
